    After breast augmentation surgery, your body forms a protective capsule of fibrous scar tissue around each breast implant. The formation of this capsule is a natural part of the healing process.

    However, for some patients, the capsule becomes abnormally thick and begins constricting the breast implant, causing the following symptoms:

    • Hard or firm breasts
    • Breasts that are painful or tender to the touch
    • Asymmetry
    • An overly round or “ball-like” appearance, which is a significant change from the pre-operative shape of the breast
    • Visible distortion or upward implant displacement

    Capsular contracture correction refers to any surgical intervention designed to release or remove the problematic breast implant capsule and restore a softer, more natural breast shape.

    Dr. Savalia emphasizes total capsulectomy, not partial removal or simple release of the breast implant capsule (capsulotomy). This ensures the elimination of all abnormal tissue and that no contaminants are left to linger in the body.

    To maximize the success of this procedure, Dr. Savalia recommends the surgical incorporation of an internal mesh bra, with choices being resorbable mesh or Acellular Dermal Matrix (ADM). Both options are often used as a preventative measure against capsular contracture.

Preparing for Your Capsular Contracture Correction

Careful planning is a priority at Savalia Plastic Surgery. Before surgery, you’ll meet with Dr. Savalia for an initial consultation. Together, you and Dr. Savalia will discuss your goals, create a surgical plan, and address any concerns or questions you have.

He will perform an examination of your breasts, evaluate rupture status, and ask you questions to understand the circumstances of your capsular contracture. He will delve into your prior surgeries, including implant exchanges or attempted corrections, adjunct materials used in previous surgeries (such as an internal mesh bra), and the type of implant device you currently have. He may request any recent imaging you have undergone for a complete picture.

At your consultation, you and Dr. Savalia will review your options at length, including:

  • Whether or not you’d like to replace your implants, and if so, implant specifications such as implant type, size, and plane of placement (subglandular or submuscular)
  • The use of synthetic or biological mesh (internal bra) to support healing and lower the risk of contracture recurrence

How Capsular Contracture Correction Is Performed

Capsular contracture correction surgery is an outpatient procedure performed under general anesthesia, much like your primary breast augmentation.

Once you are comfortably anesthetized, Dr. Savalia will make an incision to gain access to the breast implant and capsule. He then gently removes the implants and the surrounding capsule en bloc (in one unit) when possible. If your surgical plan calls for an internal mesh bra, Dr. Savalia will carefully shape and suture the selected material into place to create a reinforced pocket. If you are getting new implants, Dr. Savalia will carefully place the new implants into the new mesh-reinforced pocket. Finally, he closes the incisions. You are then moved to a recovery area and placed under close monitoring until you wake from anesthesia and are ready to be driven home.

Additional procedures, such as fat grafting or a breast lift, may be performed at the same time as your corrective surgery, or they may be scheduled for a future date after you have healed. You and Dr. Savalia will discuss which is best for you at your consultation.

  • What Is the Recovery Like From Capsular Contracture Correction?

    Many of Dr. Savalia’s patients note that their recovery from capsular contracture correction is far easier than their initial breast augmentation recovery. Many patients can return to work after about a week. Light activity, such as walking and showering, is usually permitted a few days after surgery. Swelling will resolve in a few weeks, and postoperative pain and discomfort can be managed with a combination of TYLENOL® and Advil—Dr. Savalia’s breast surgery patients rarely need anything stronger.
